Enrolling or transferring an individual to a new HCS provider

IWhen an individual chooses to enroll or transfer to a Home and Community-based Services (HCS) provider, the provider must notify the individual's service coordinator at the local authority. A new HCS provider who has their first individual in their program should contact the PE/UR Message Line at 512-438-5055.

A program advisor will be assigned to the provider and the provider will be informed of the assigned program advisor's name and contact information. The assigned program advisor will contact the provider to assist with opening the provider's 1st Location Code in the CARE system. Upon authorization of the individual's enrollment or transfer, the assigned program advisor will notify the DADS Provider Contracts Unit. The DADS Provider Contract Unit will assist the provider with obtaining access to the CARE system.

Notification of information and policy letters and other DADS information

DADS notifies program providers of changes to policy, procedures and forms by posting the information on the DADS website. You can be notified of these postings by registering for the Gov Delivery email notification system.

Always be sure that you are using the current DADS forms. The current forms are available on the DADS website. Select the program type for which you need forms, click on the program link, and click on the forms link within the program type.
